If a new identity is added the dialog asking for the "Identity Name"
should either default the realm or force the user to enter a realm.
Might be nice if it included the realm drop-down like in the Obtain new
credentials dialog.

Problem is the "Add new identity" allows you to create an identity without
a realm and that identity hangs around.

FYI. This MIGHT (and I stress MIGHT) be related to bug I saw ONE time
where the Obtain new credentials dialog got stuck. Even after entering
the password and getting credentials the dialog didn't go away. I haven't
been able to repeat this, so I haven't sent a bug in on it.

We agree that the Add new identity dialog needs to be better.
We needed to add something in order to allow identities to be
added without obtaining credentials. I am going to leave this
open with the hope that we can get to it for KFW 3.2.

In the meantime, we should generate an error if no realm is specified.
